INSURANCE REQUIREMENTS
To be listed as covered:

Equipment:
1.) Commercial General Liability - Must have a minimum of $1,000,000 (1 Million) in coverage.
2.) Miscellaneous or leased equipment for replacement value.
3.) Chism Grip, Inc. should be named as Additional Insured as their interests may appear.
Vehicles:
1.) Commercial General Liability $1,000,000.00 each occurrence minimum.
2.) Automobile Liability $1,000,000.00 Combined Single Limit - Must include “Hired / Non Owned Autos”
3.) Physical Damage (Comprehensive and Collision) - Limit no less than $100,000.
4.) Rented Equipment Coverage ($15,000.00 minimum). Must show deductible amount (not required for empty vehicle rental).
5.) Chism Grip, Inc. should be named as an Additional Insured and as Loss Payee as their interests may appear.
